We collect personal information you provide through enquiry forms, referrals, service agreements, and clinical documentation. This may include: name, contact details, participant condition and care needs, NDIS plan details, and any clinical information you choose to share with us at intake.
How we use it
We use your information to respond to enquiries, deliver NDIS supports, document services for plan reviews, and communicate with treating clinicians and Support Coordinators where authorised. We do not sell information to third parties.
Storage and security
Information is stored on secure systems with access restricted to authorised WEYA staff. Clinical documentation is retained in line with NDIS Practice Standards and relevant Australian record-keeping requirements.

Complaints
